  1. Contestants may enter the Contest by completing the official entry form and submitting it on the Cosmetic Laser MD website.

  2. The Cosmetic Laser MD Competition is open to any citizen and/or resident of the United States (and its territories and possessions) who, as of January 1, 2010, is above the age of 18 at the time of entry. Prize winners of past Cosmetic Laser MD competition(s) and employees of Cosmetic Laser MD, affiliates and subsidiaries are ineligible.

  3. The likelihood of winning will depend upon the accuracy of all other entries, as the finalists with the CORRECT answer will be put into a lottery drawing to win. Prizes are nontransferable. All prizes will be awarded. Competition is void where prohibited or restricted by law. Winners will be solely responsible for any federal, state or local taxes. Within 10 days of the date specified in the notification, winners will be required to execute an affidavit of originality, eligibility, and compliance with the rules, a release of liability and grant of publication rights, or the prize will be awarded to an alternate winner.

  4. All answers and all rights to their publication become the property of Cosmetic Laser MD and Network Design and Communications , which may use, edit and excerpt these entries for promotional or any other purpose, including placing them online (e.g., on www.cosmeticlasermd.com) without attribution or compensation, additional or otherwise. Cosmetic Laser MD and Network Design and Communications reserve the right to use prizewinners' names and hometowns in any advertising or promotional materials relating to this competition without further notification, permission, or compensation to the winners, except where prohibited by law.

  5. Finalists will be selected during the week following the competition, July 5th to the 7th, and will be notified by e-mail. Winners will be determined based on a random lottery pulling and be posted online at Cosmeticlasermd.com and the Cosmetic Laser MD Facebook Fanpage.

  6. Use of entrants and winner information. The information that you provide in connection with this contest may be used by the Sponsors (Cosmetic Laser MD and/or Network Design and Communications) to send you further information about its products and services. If you would like your name and information to be removed from the Sponsor's mailing list please be sure to unclick the “Subscribe me to your Newsletter” box at the bottom of the entry form.   7. DISCLAIMER: Sponsor is not responsible for technical, hardware or software malfunctions, lost or unavailable network connections, undeliverable email, or failed, incorrect, inaccurate, incomplete, garbled or delayed electronic communications, telephone, telephone line or other telephone related problems or errors, whether caused by the sender or by any of the equipment or programming associated with or utilized in this Contest, or by any human error which may occur in the processing of the entries in this Contest. If, in the Sponsor’s opinion, there is any suspected or actual evidence of tampering with any portion of the Contest, or if technical difficulties compromise the integrity of the Contest, Sponsor may in its discretion terminate the Contest entirely, determine a winner from among all eligible entries received as of the date of termination, and/or to modify the Contest as Sponsor deems appropriate. If there is a question about the identity of an online entrant, the entry will be deemed made by the owner of the email address identified on the entry.

  8. No substitutions for prize. Prize is non-transferable, cannot be redeemed for cash. Liability of taxes and any other associated costs are the sole responsibility of prize winner. All prize awards are final.
